overheating problem

hey guys got a slight problem.... after driving for a while my heat and everything works amazing.. than i start overheating usually on the highway in 5th gear at a constant speed.....

lower rad hose is freezing cold.... ive replaced t stat 3 times... cause the inlet tube is hot too till i get to the t stat than the lower rad hose is freezing...

anyone have any suggestions???

u sure u have coolant?
r u leaking coolant?
is the coolant any good?

maybe the rad is plugged/blocked?
