well, a twin within a twin has nothing to do with a health care system. parasitic twins occurs in every sort of population. there's nothing you can do about it i.e. there is no treatment to prevent it...it just happens. north america has tonnes of birth "defects"...birth defects are prevalent in every society. this case got much media attention b/c of the observable phenotype associated with this particular parasitic twin situation.
