could be many things, from bad EGR, carbon fauled IACV, or throttle body, bad plugs, leaking vacuum, air in coolant. Start by doing some damn basic maintenance. Flush and fill coolant. Test. Clean throttle body, test. Change plugs and wires, test. Remove and inspect EGR (if applicable, I dunno whether EDs have EGR systems). Replace PCV as well, hook up vacuum gauge and test vacuum. Need good, steady vacuum.

no because there would be no back pressure ... but I drove my car like that till I could go pick up a new one and it was fine....

seriously do the basic maintenance Bruno suggested and put a muffler on should be fine...
