Bigger engine problem!

So I have a friend who is like Simple Jack when it comes to cars.
He has a US 99 Civic EX (cdn Si) He has had it for about a year 1/2 and has driven around with low oil (or no oil) puts crap no name gas in and almost zero maint.
In Aug his car was sounding a bit crap and I did not get to look at it.
The next day he drove from Toronto to the Rockys and back.
The day he gets back and tells me the car can not rev up past lets say 4gs (I cant remember) and it takes ages to get up to 100km and wont go much past.
So a friend and I do a bit of work on it:
Check the plugs, fine
Change his crap ram air intake.
We find that the exhaust is cracked in a few spots and is just hanging there.
Then we change the fuel filter and the car still sounds like *** but it revs fine and gets to 100 plus no problem.
A few weeks/days go by same things happens but my friend is in London On so I cant really look at his car.
He thinks its just because of the exhaust but that cant be it.
I picked up a new one for him so that will be installed tomorrow.
Any ideas of what it could be? I was thinking the injectors, maybe one of the O2 plugs or maybe even the cam position sensor?

Sorry about the ramble but any ideas would help (hes too cheap/simple to take it to a shop)

who cares?
We're all here to help.

It sounds like it could be a problem with the distributor.
