That sounds so much like a vac leak.

I found the following from another forum, this guy has the same engine and same problem your having:
---------------------------------------------------------------
"I thought it was the IACV, I replaced it, and it didn't fix ****.

Ran a diagnostic. It was something starting with my IACV, going through the sensors, to the wires, to the ECU.

Found a buddy with a 98 DX 5SPD, just a hatch, not like my 2dr :) popped in his ECU, idle was perfect.

I'd really recommend at least trying someone elses ECU it takes literally 2 seconds, and it fixed my problem after months of trouble shooting. "
---------------------------------------------------------------

Why don't you find a stock ECU and swap it. It might actually work.

def check the fitv.. its located under the TB check honda tech for a write up there is a good one there..

for future knowledge idle issues that are caused by the IACV will only effect the car once its at running temp.
